Please enter a name for your work in the "Document/Image Name" section, (for example, if your name is Susan Lewenz, enter "susan_lewenz" -- no quotes or spaces).
Click the first "Browse/Choose File" button to upload your image in "JPG or GIF" format.
Click the second "Browse/Choose File" button to upload the doc file containing the details of your work and yourself (media, dimensions, contact information).
